Boeng Trabek-NCD Community Management Project-PMCI Report on Blood Sugar Test (BST) and High Blood Pressure (BP) Test

Venue: Boeung Trabek Slum Community (Village 1, Village 4, and Village 5),Sangkat Phsar Derm Thkov, Khan Chamkar Morn, Phnom Penh, Cambodia
Purpose :To verify the real health data of previously interviewed patients who reported having Non-Communicable Diseases (NCDs), such as diabetes and hypertension.
Expected Outcome
▪ Verified patient data on NCDs (Blood Sugar and Blood Pressure).
▪ Improved understanding of NCD prevalence within the Boeung Trabek community.
▪ Strengthened patient follow-up and data management for future health interventions.
Challenges
▪ Limited time to cover all target households.
▪ Some patients were absent and raining season or working during the survey hours.
▪ Lack of consistent health records among community members.
